Clinical case: Urinary pain and hematuria in a 5-year-old child treated for leukemia
Boy, 5 years · Emergency department · Oncology
Adam, a 5-year-old boy, is being followed for standard-risk acute lymphoblastic leukemia (ALL) with t(12;21) translocation. He has been in the maintenance phase of his chemotherapy for two years. Three weeks ago, he presented with a productive cough and intermittent fever: cytomegalovirus (CMV) pneumonia was diagnosed and treated with ganciclovir and broad-spectrum antibiotics.
How a case is played
No multiple choice questions: a patient, a ward room, a consulting room or a counter, and your decisions.
-
You question them
Out loud or in writing. The patient answers in their own words, and only tells what they are asked.
-
You examine them
Auscultation, palpation, neurological examination: you choose what to examine, and the finding follows what you look for.
-
You decide
Tests, treatments, procedures: the nurse carries out your orders, and the patient responds.
-
You understand
The debrief goes back over each decision with its reason, and your weak points become practice missions.
